The monies to be paid on application is USD $141.00.
Broken down as follows:
1. Joining (One-Time) - USD $5.00
2. Charity Call (Annual) - USD $15.00
3. Administration (Annual) - USD $12.50
4. Insurance Premium (Annual) - USD $108.50
